Checklist for initial compliance under the Code

Before filling out the Disclosure Statement for Members and their Family Members, please make sure you have the following information about your private interest and the those or your family (spouse and dependent children):

1. A detailed list of all your assets, including assets held jointly with another individual and business assets, as well as an estimate of their value.
2. A detailed list of all your investments. You are required to provide details on all your investments, whether held directly or through a financial institution, a brokerage firm or an online trading company. Please include a copy of your most recent statements for each account, as well as certificates or other investment documents.
3. A detailed list of all other personal assets, including whole life insurance.
4. A detailed list of all the income you received in the last 12 months and the income you are entitled to receive in the next 12 months.
5. A detailed list of all your liabilities, including the amount of each liability, enclosing a copy of your most recent statement for each liability.
6. A detailed list of all the activities in which you participated in the two years prior to your election.
