As a Member of the House of Commons, you must fill out this form as part of your initial compliance process under the Conflict of Interest Code for Members of the House of Commons, no later than 60 days after notice of your election is published in the Canada Gazette.
The form asks for detailed personal and financial information about you and your family members. The form must be submitted with supporting documents, such as financial statements. Please consult the Checklist for initial compliance under the Code for a list of information you will need on hand. You can expect to spend at least an hour to fill it out. If you need assistance to fill out the form, an advisor in the Commissioner’s Office can help.
You can send the completed form to the Commissioner’s Office by email. An advisor will contact you to guide you through the rest of the initial compliance process.
If you are a minister or parliamentary secretary, you must follow a similar process under the Conflict of Interest Act.
